Purpose:To compare the outcome of vitreous surgery with 20- and 25-guage system for epiretinal membrane and macular hole. Cases:This retrospective study involved 50 eyes operated during the past 9-month period. We used 20-guage system in 28 eyes and 25-guage system in 22 eyes. Phacoemulsification-aspiration with intraocular lens implantation was performed in 18 eyes in the former group and in 10 eyes in the latter. All the eyes received triamcinolone. Indocyanine green was used in all the eyes of macular hole. Results:Duration of surgery was significantly shorter when the lens was removed in eyes of epiretinal membrane(p=0.02)and when the lens was left intact in eyes of macular hole(p=0.04). Postoperative complication comprised ocular hypertension 18% when treated by 20-guage system. Ocular hypertension 9%,hypotension 23%,choroidal detachment 18%,and retinal detachment 5% when treated by 25-guage system. Conclusion:Use of 25-guage system results in shorter duration of surgery and in higher incidence of postoperative complications.
