長崎大学における25Gシステムと20Gシステムの硝子体手術成績の比較

Outcome of vitreous surgery using 20-guage and 25-guage systems

目的：黄斑前膜と黄斑円孔に対する硝子体手術で，20ゲージと25ゲージを使った成績の比較。症例：過去9か月間に硝子体手術をした20ゲージによる28眼，25ゲージによる22眼を対象とした。前群では18眼，後群では10眼に水晶体乳化吸引術と眼内レンズ挿入術を併用した。全例でトリアムシノロンを使用し，黄斑円孔ではインドシアニングリーンを使用した。結果：25ゲージによる手術時間は20ゲージよりも，黄斑前膜では水晶体摘出群（p＝0.02），黄斑円孔では硝子体手術単独群（p＝0.04）で有意に短かった。術後合併症として，20ゲージ群では高眼圧18％，25ゲージ群で高眼圧9％，低眼圧23％，脈絡膜剝離18％，網膜剝離5％が起こった。結論：25ゲージシステムを使うことで手術時間が短縮できるが，術後合併症の多いことが問題である。

Purpose：To compare the outcome of vitreous surgery with 20- and 25-guage system for epiretinal membrane and macular hole. Cases：This retrospective study involved 50 eyes operated during the past 9-month period. We used 20-guage system in 28 eyes and 25-guage system in 22 eyes. Phacoemulsification-aspiration with intraocular lens implantation was performed in 18 eyes in the former group and in 10 eyes in the latter. All the eyes received triamcinolone. Indocyanine green was used in all the eyes of macular hole. Results：Duration of surgery was significantly shorter when the lens was removed in eyes of epiretinal membrane（p＝0.02）and when the lens was left intact in eyes of macular hole（p＝0.04）. Postoperative complication comprised ocular hypertension 18％ when treated by 20-guage system. Ocular hypertension 9％,hypotension 23％,choroidal detachment 18％,and retinal detachment 5％ when treated by 25-guage system. Conclusion：Use of 25-guage system results in shorter duration of surgery and in higher incidence of postoperative complications.
