A 76-year-old man was referred to us for normal-tension glaucoma since 8 years before. His corrected visual acuity was 0.8 right and 0.9 left. Both eyes showed notching and nerve fiber layer defect temporal to the disc. Visual field in the left eye was classified as Ⅱ-b after Kosaki. He revisited us for pain and progression of visual field defect in his left eye. There was no change in fundus findings or visual acuity. The left eye showed paranasal and temporal-superior scotoma. Central critical fusion frequency was decreased. Magnetic resonance imaging(MRI)showed a localized area of high intensity in the left optic nerve. Laser speckle showed reduced blood flow in the optic disc and the surrounding retina. He was diagnosed with presumed posterior ischemic optic neuropathy in addition to normal-tension glaucoma.
