Abstract Objective:We report two cases of macular hole associated with retinitis pigmentosa treated by vitrectomy.

Case 1:A 64-year-old man was referred by his local physician due to vision loss in the right eye. Right visual acuity was 0.05(0.1). Bone spicule pigmentation in the periphery of the fundus was observed in both eyes. Goldmann's visual field showed concentric contraction in both eyes. The erectroretino graphy(ERG)was flat in both eyes. Optical coherence tomography(OCT)findings showed a large macular hole(578 μm diameter)in the right eye. Phacovitrectomy using a head-up surgery system and modified inverted internal limiting membrane(ILM)flap-technique were performed, and SF6 was injected. Six months after surgery, the macular hole was closed and visual acuity improved to 0.5.

Case 2:A 43-year-old woman was referred by her local physician due to decreased vision in the left eye. Left visual acuity was 0.3(0.4). The fundus showed osteoid pigmentation in both eyes except in the macular area. Goldmann's visual fields showed afferent visual field stenosis and mottled visual field defects in both eyes. The ERG was flat in both eyes. OCT showed a macular hole(284 μm diameter)in the right eye, and phacovitrectomy was performed in the left eye. The ILM was stripped, and SF6 was injected. Six months after the surgery, left visual acuity improved to 0.8, and OCT showed closure of the foramen magnum.

Conclusion:The head-up surgery system is useful for reducing phototoxicity in retinitis pigmentosa, and the modified inverted ILM flap-technique is effective in refractory cases.
