白内障手術中の散瞳維持効果を,ジクロフェナクナトリウム(ジクロフェナク)とブロムフェナクナトリウム(ブロムフェナク)について比較した。両眼が同程度の白内障6例12眼に同一術者が手術を行った。手術の2時間前からトロピカミドとフェニレフリンを15分の間隔で点眼し,さらに一眼にはジクロフェナク,他眼にはブロムフェナクを30分間隔で点眼した。手術は耳側透明角膜切開による超音波乳化吸引術と眼内レンズ挿入術で行い,瞳孔径を計測した。灌流液にはエピネフリンを添加しなかった。皮質吸引終了までの縮瞳率,手術終了時の最終縮瞳率には,ジクロフェナク群とブロムフェナク群とで有意差がなかった。ブロムフェナクの術前点眼は,従来から使われているジクロフェナクと同等の効果を示した。
We evaluated bromfenac and diclofenac ophthalmic solutions regarding the antimiotic effect during cata-ract surgery. A single surgeon operated on 12 eyes of 6 patients. Each eye received instillation of tropicamide and epi-nephrine every 15 minutes starting 2 hours before surgery. Additionally, diclofenac was instilled into one eye every 30 minutes. Bromfenac was similarly instilled into the fellow eye. Surgery was performed by temporal clear cornea inci-sion, phacoemulsification and aspiration, and intraocular lens insertion. Epinephrine was not added to the irrigating so-lution. The pupillary diameter was monitored at regular intervals. There was no significant difference between bromfe-nac and diclofenac groups regarding the rate of change in pupil size either at the end of aspiration of lens cortex or at the end of surgery. The findings show that preoperative instillation of bromfenac is as effective as diclofenac in their antimiotic effects.
