9歳男児に認められた網膜中心動脈閉塞症の1例

Central retinal artery in a nine-year-old boy

　9歳男児が起床時に左眼視力低下を自覚し，その翌日受診した。左眼視力は手動弁であり，眼底に典型的な網膜中心動脈閉塞症の所見があった。蛍光眼底造影で網膜循環時間の遅延があった。当初は血液学的に異常がなかったが，1か月後に抗リン脂質抗体（抗カルジオリピンlgM抗体）が陽性化した。小児の網膜中心動脈閉塞症は稀であるが，抗リン脂質抗体症候群と関係して起こる可能性があることを示す症例である。

A 9-years-old boy presented with sudden visual loss in his left eye since the day before. His left visual acuity was hand motion. Funduscopy showed typical findings of central retinal artery occlusion. Fluorescein angio-graphy showed delayed intraretinal circulation. Systemic examinations, including hematological studies, were initially normal. One month later, he was found positive for anticardiolipin antibody. This case illustrates that central artery occlusion may develop in a child with antiphospholipid antibody syndrome (APS).
