Purpose:Inhabitants in Okinawa have a high incidence of shallow anterior chamber. This feature may mean a higher risk of corneal endothelial damage during cataract surgery. We evaluated the value of soft-shell technique in protecting the corneal endothelium. Cases:This retrospective study involved 450 eyes that received cataract surgery in the past 4 years. All the eyes were examined for corneal endothelial cells before and 3 months after surgery. Soft-shell technique was used in 181 eyes and cohesive viscoelastic substance in 269 eyes. Results:Corneal endothelial cells decreased by 8.6% in soft shell group and 16.2% in viscoelastic group. The difference was significant(p=0.02). Similar findings were obtained in 182 eyes with shallow anterior chamber,and in 422 eyes with nuclear hardness ranging from grade 1 to 3 after Emery-Little. Stress index was 31.1 in soft-shell group and 47.1 in the other. The difference was significant(p=0.03). Conclusion:Soft-shell technique protects the corneal endothelium during cataract surgery in eyes with shallow anterior chamber and in eyes with nuclear hardness ranging from grade 1 to 3.
