　外側型の肘離断性骨軟骨炎（以下肘OCD）に対して大腿骨内顆から骨軟骨ブロック移植術を行った3例につき報告する．症例は2例が野球選手，1例はバドミントン選手である．骨軟骨をブロック状に採取し，肘欠損部へ移植してスクリューで固定した．3症例とも肘・膝に愁訴なくスポーツに復帰している．外側型肘OCDに対して，硝子軟骨による関節面と安定した外側壁を同時に再建可能な本術式は有用な方法と考える．

　We report three cases of the lateral type of osteochondritis dissecans of elbow (elbow OCD) in advanced stage, and introduce new osteochondral shell graft technique. Surgical procedures included havesting “blocky” osteochondral shell graft containing articular surface and cortical bone from femoral condyle, and curetting the lesion at humeral capitellum, and then fixing the graft with screw. We performed this procedure for two baseball players and one badminton player, and all of them returned to their previous sports. We think this procedure is a useful method for the lateral type of elbow OCD.
