　肩関節脱臼骨折に対する人工骨頭置換術中の脱臼骨頭摘出時に生じた動脈損傷を経験したので考察を加え報告する．症例は上腕骨人工骨頭置換術を施行した4 part肩関節脱臼骨折の2症例である．2例とも術中，骨頭の摘出の際に肩甲下動脈からの動脈出血を認め，止血困難となった．Neer分類で前方脱臼骨折での骨頭の整復および摘出の際には，受傷時に損傷した動脈からの出血や，不整な骨折面による動脈損傷を起こすリスクがある．2症例の経験からも，出血後の術野からの止血は困難で，われわれ整形外科医による処置にも限界がある．その認識と予防が大切と考え，また動脈損傷を念頭に置き血管外科医など専門医の確保が必要と考える．

　We report two cases of intraoperative arterial injury during surgery for fracture-dislocation of the shoulder. The patients were a 70-year-old and a 74-year-old male with 4 part fracture-dislocation of the shoulder. Both patients underwent partial prosthetic replacement of the shoulder. After the humeral head was removed, bleeding from the injured subscapular artery persisted and could not be arrested. There is a risk of bleeding from injured artery in fracture-dislocation of the shoulder. Specialist, such as vascular surgeon must be secured before the operation.
