　脳梁前半部離断のみで十分な発作改善がなかった難治性全般発作5例に，二期的脳梁全離断を行った。対象は全例，軽〜重度の精神遅滞があり，前半部離断時の年齢は平均10.2歳で，4例が小児期に手術を行った。発作型は転倒発作（DA）3例，強直発作（TS）2例，ミオクローヌス（MY）2例，複雑部分発作（CPS）2例，非定型欠神発作（AA）1例，頭部前屈発作（HD）2例に認められた。前半部離断後にCPSが1例で消失，DAは1例で50〜80％消失，他の発作はいずれも50％以下の減少率であった。一方，平均2年後の後半部離断術後にはDAは全例で消失，AAの1例は80％以上消失した。脳梁離断症状は成人の1例に出現したが，習慣性のある日常動作に支障はなかった。脳梁前半部離断で抑制が困難な転倒発作に対して後半部離断は有効であった。特に小児では術後合併症はきたしにくいため，なるべく早期に後半部離断の可能性を検討するべきと思われた。

For the treatment of intractable generalized epilep-sies, two-staged total callosotomy was performed in five patients. In all patients, preceded anterior cal-losotomy failed to obtain satisfactory seizure control. All patients showed mental retardation with various degrees. Mean age at the first operation was 10.2 years and 4 patients were operated in their childhood. All patients showed various types of seizures ; drop at-tack (DA) in 3 patients, tonic seizure (TS) in 2, myo-clonus (MY) in 2, complex partial seizure (CPS) in 2, atypical absence (AA) in 1, and head drop (HD ) in 2.
