正中神経と尺骨神経の運動遠位潜時の比(DLR)は,健常者での1.29±0.10に対して,糖尿病患者303名では1.44±0.24に増大した。DLRが健常者の上限である1.49を超えた糖尿病患者は,93名(30.7%)に達し,とくに女性では39%を占めた。下肢での多発ニューロパチーの進行度を表す多発神経障害指数(LPNI;正常100%)を5%ごとに区分し,DLR異常者の頻度を検討したところ,LPNIが低いほどDLR異常者の割合が増加した。以上から,糖尿病患者では手根管部伝導障害が多くみられ,かつ多発ニューロパチーの進行とともにその頻度が増すことが示された。このように,多発ニューロパチーの存在下で局所的神経障害の頻度が増す現象も,一種の重複神経障害として捉えられると考える。
We investigated 303 diabetic patients in order to clarify the relationship between progression of dia-betic polyneuropathy and conduction delay across the carpal tunnel. Distal latency ratio (DLR) was determined by comparison of distal motor latency of the median nerve with that of the ulnar nerve. Lower extremity polyneuropathy index (LPNI), expressed as a mean percentage of the normal for six indices over two nerves obtained by motor nerve conduction studies, was 82.9% on the average in the patients. Their DLR (1.44±0.24) was larger than the normal value (1.29±0.10).
