糖尿病性ニューロパチーの評価に感覚神経活動電位(SNAP)を用いることの妥当性を知るために,253名の糖尿病患者で正中・尺骨・腓腹神経から逆行法を用いて得たSNAPの振幅と伝導速度につき,指標相互および運動神経伝導速度に関する12指標の健常者平均に対する比(PNI)との相関を検討した。正中神経と尺骨神経のSNAP振幅間の相関は,他の神経の組み合わせよりも高かった。一方,糖尿病性ニューロパチーの程度をよく表現するPNIとの相関関数は,振幅で0.4前後,伝導速度で0.55前後であった。しかし,3神経の平均値をとると,PNIとの相関係数が振幅で0.48,伝導速度で0.60に上昇した。1神経のみのSNAPには,個人差や部位差あるいは測定誤差が大きく影響する。しかし,3神経の平均値を用いることにより,全身に生じる糖尿病性ニューロパチーの程度をより的確に表現する指標になるものと考えられた。
In order to clarify the suitability of sensory nerve action potential (SNAP) in the evaluation of diabetic polyneuropathy, we studied measurements of SNAPs in the median, ulnar and sural nerves. Subjects were 253 patients with non-insulin dependent diabetes mel-litus ; 167 men and 86 women, aged 58.2 ± 12.8 (mean ± SD) years old. Their diabetic history was 10.2 ± 8.6 years. SNAPs were recorded antidromically from in-dex finger, little finger and lateral to the Achilles ten-don, respectively. Twenty-eight patients, in whom any one of the SNAPs couldn't be obtained, were already excluded from this study.
