　臨床症状および経過からはOPCAと鑑別が困難であった特発性脳表hemosiderin沈着症の1例を報告した。MRI，IMP-SPECT，脳槽シンチグラム，ABR，SEPなどの画像・電気生理学的所見を検討した。症例は59歳の男性で，緩徐進行性の小脳性運動失調，排尿障害，難聴，痴呆を来した。頭部CT上の小脳と脳幹の萎縮の所見から脊髄小脳変性症が疑われたが，MRIのT₂およびプロトン強調像にて脳・脊髄表面に特徴的な低信号域が認められたため，臨床像と合わせて脳表hemosiderin沈着症と診断した。IMP-SPECTでは，異常MRI信号部位と一致して脳血流の低下がみられ，脳槽シンチグラムでは髄液の停滞を認めた。ABR，SEPでは，中枢伝導時間の遅延を認めた。明らかな出血性基礎疾患を有する二次性脳表hemosiderin沈着症に比し，系統変性症的病像を呈する特発性脳表hemosiderin沈着症の病態の評価にこれらの検査は有用と考えられた。

A 59-year-old man developed a staggering and wide based-gait in July 1990. Dysarthria, hearing loss, vexation and disturbance of memory appeared in January 1991. He consulted our clinic in May 1991, and cerebellar ataxia, neurogenic bladder, and cerebellar atrophy on brain CT were noted. Subse-quently, he was followed as OPCA. Brain and spinal cord MRI (T₂ and proton weighted images) revealed hypointensity on the surface of the Sylvian fissure, cerebellum, brainstem and spinal cord.
