新抗てんかん剤Tegretolのてんかん発作ならびにてんかん性精神障害に対する効果

TREATMENT OF EPILEPTIC SEIZURES AND PSYCHIC DISTURBANCES WITH TEGRETOL, A NEW ANTIEPILEPTIC AGENT

　われわれはTegretol，すなわち5—carbamyl−5H-di—benzo　〔b, f〕　azepinを難治てんかん患者46名に投与してその効果を検討した結果，本剤はてんかん発作に対して広い治療スペクトルムを持つのみならず，従来治療の困難であつたてんかん性精神障害のうち，てんかん性の性格障害や精神病様症状にもかなりよく奏効する綜合抗てんかん剤であることを認めた。

　1）　まずてんかん発作に対しては，とりわけ大発作，精神運動発作に奏効する。その仙，焦点発作，精神発作，失立発作，自律神経発作にも奏効例があるが，小発作に対しては効果がなく，時には増悪させた例もあつた。

　2）　てんかん性性格のうち最も奏効しやすいのは，易怒，爆発性，不機嫌などの感情不安定性であり，症例の半数以上に効果を認めた。大発作をもつ症例のてんかん性格に最も奏効しやすく，ついで粘神運動発作型，大発作と精神運動発作の混合型の順に奏効した。

　3）　従来の抗てんかん剤では，ときとして抗発作作用と向精神作用との相反関係が認められ，発作の抑制された症例にかえつて精神症状の悪化する場合を経験したが，本剤にはそのような傾向はほとんど認められず，この両作用が平行関係にあるように思われる。

A new antiepileptic agent, Tegretol, with the for-mula: 5-carbamyl-5H-dibenzo (b, f) azepin, has been tried on 46 epileptic patients.

The results obtained were as follows:

Tegretol was effective not only in various types of epilepsies but also in the epileptic psychic disturb-ances, for example, epileptic personality changes and epileptic psychosis whose medical treatment has been difficult up to the present.

The effect of the drug was especially prominent in the patients whose seizure types were grand mal or psychomotor seizure. It was found in the type of focal seizures, psychic seizures and autonomic seizures, but not in the petit mal type patients.

Improvement of the epileptic psychic disturbances was demonstrated. It was greatly effective in treating emotional instability.

Correlation between the clinical effects and impro-vement in EEG was not always observed.
