頭部外傷とヘルメツト

MECHANICAL STUDIES OF HELMETS IN HEAD INJURIES

I．まえがき

　最近の頭部外傷の増加は，産業災害や交通災害を中心にいちじるしいものがあり，ひとつの大きな社会問題にまでなつてきている。この対策として，労働基準法等により作業設備や安全管理の体制整備，安全教育の徹底等を計つており，また一方では交通法規の改制強化等で大童のようである。しかも事故発生はかなり不可避のものもあるようで，なお増加の一途を辿つており，法規の力もこれに抗しきれないような現状である。このような状況から，自分自身を守り，傷害を最少限に止めようとヘルメットが着用され始めたが，最近では一段と普及し，一部ではその着用が法規化（労働安全衛生規則第129条の2）されヘルメットそのものもかなり厳重に規格化^1）2）されてきている。最近，ビル工事現場に，オートバイの乗者にと，ヘルメットは日常身近に感じられるようになつてきた。しかし，このようにわれわれに卑近なものになつてきたヘルメットについて，その効果を報告した文献は非常に少なく，多少性質を異にするが旧陸軍の大迫³），山崎らの報告がわずかにみられるにすぎずこれを数量的に示したものは海外にもみられない。

　著者は，以下に頭部外傷を起こす物理学的因子のうち，1）衝撃エネルギー，衝撃時運動量および衝撃速度の3因子について，意識障害の長さ，頭部レ線所見，脳波所見との関係を追求し，いずれの因子がこれらの臨床像と最も密接な関係にあるかを決定し，2）その因子で現在使用されている作業用ヘルメットの効果をある程度数量的に明確化し，あわせて乗車用ヘルメットとも比較検討しヘルメットの優劣についても検討する。

(1) The author reviewed 154 cases of head injury caused by falling on the head or being struck on the head with a falling object, since these kinds of trauma are relatively sim-ple from a dynamical point of view and take place more frequently in comparison with head injury due to other causes.

The author studied the velocity with which a patient was struck, the momentum and the mechanical energy, and correlated these factors with the duration of impaired consci-ousness, the roentogenographic findings ofthe skull and the E. E. G. findings. It was con-cluded that the impact energy was the one which was correlated most closely with the clinical findings.

(2) The helmet may be the only possible tool provided against head injury. Wearing a helmet has become more popular and genera-lized. There can be found, however, few literature about the effect of a helmet based on a medical point of view. The author studied the effect of a helmet for head injury in view of the "energy" which is the most correlative factor with the clinical findings and found that the difference of the impact energy between cases with and without a helmet having the same clinical symptoms, corresponded approximately 500 to 1,000 joules. The value, however, varies according to the types of the helmet, especially to the quality of a cushion inserted between the helmet and head.
