　症例は20歳の女性で，嚥下困難，胸痛と左腕のしびれを認め，1990年8月23日当院を受診した．胸部X線写真で心臓に沿った透亮像を認め，縦隔気腫を疑い，さらに胸部CTを施行した．CTでは気管，胸部大動脈および心臓周囲にfree airを認め，特発性縦隔気腫と診断した．治療は安静のみで8日間で治癒した．本例では腕のしびれが特徴で，その機序としては縦隔内圧上昇により腕への血管が圧迫され血流障害を起こしたためと考えられる．

A 20-year-old female developed dysphagia, chest pain and numbness of the left arm after dinner on August 22, 1990. She consulted a doctor, who diagnosed tachycar-dia and prescribed propranolol. On August 23, she hadn't improved. On the same day, she visited our hospital. Chest X-ray films revealed lucent band and distinct line running parallel to the border of the heart and we suspected mediastinal emphysema. Immediately we took chest CT scan, which demonstrated free air surrounding the trachea, thoracic aorta and heart. We diagnosed spontaneous mediastinal emphysema.
