　肺線維症に多発性気腫性肺嚢胞を伴ったvon Reck-linghausen病は1963年Daviesらにより報告されて以来^1），欧米では数多くの報告がある^2〜14）。本邦では数例の報告^15〜17）があるが，胸部X線，胸部CT，肺機能，病理所見の特徴を包括的に報告したものはない。

　今回我々は典型的な皮疹を有し，胸部X線上間質性肺病変を示す網粒状影を認めたvon Recklinghausen病を経験した。胸部CTでは間質性陰影に加えて多発性肺嚢胞が描出された。肺機能検査上は肺気量の増加，静肺コンプライアンス増加並びに呼出性換気機能障害とむしろ肺気腫に対応する検査所見を示した。経気管支肺生検（TBLB）では肺線維症と肺気腫の混在が確認された。本症の肺病変の特徴として線維化性病変と気腫性病変が共存することが考えられた。

A 51 year old man was diagnosed as hanving von Recklinghausen's disease from his skin lesions.A chest film showed reticulonodular shadows sug-gesting the interstitial lung disease. A chest computed tomography showed multiple emphysematous pulmo-nary bullae in addition to the interstitial shadows. Mild airway obstruction was observed in pulmonary function tests, while subdivisions of lung volumes and capacities, namely the vital capacity, the func-tional residual capacity and the residual volume all increased. The static lung compliance also in-creased to 0. 35 l/cmH₂O.

The pathological findings obtained from trans-bronchial biopsy revealed both fibrosis and emphy-sema. We have reported this case because of the presence of the characteristic features in the pul-monary lesions with von Recklinghausen's disease regarded as co-existence of both emphysematous and interstitial changes.
