虚血性心疾患に対するpropranolol trapidil併用の効果—運動耐容能および心機能に関する研究

The effects of propranolol and trapidil to ischemic heart disease: evaluation of their action on cardiac function and exercise tolerance test

　虚血性心疾患の治療薬の薬理作用による分類は種々な面からされているが，橋本の提唱のごとく^1），心機能との関連で分類すると，1）心機能を低下させるもの，2）心機能に影響しないもの，3）強心効果を持つものに分類される。実際の治療にあたっては，症例に応じこれを使いわけたり，または併用する必要がある。β—blockerは1）の作用を持つものとして知られており，一方3）の作用を持つものとしてはtrapidilが知られている。この薬剤は多様な薬理作用を持つが，冠拡張作用以外にtheophyllineやpapaverineに類似した強心作用を持ちこれはβ—blockerでは抑制されないとされている^1,2）。しかしこの点を検討した臨床データーはみられていない。我々はこの作用を臨床的に検討する目的で，既知の1）の作用を持つβ—blockerを投与後にtrapidilを併用することにより，臨床的には明らかでない強心効果が明確になることを予測し，心機能を非侵襲的に測定するとともに，両薬剤が冠循環に与える効果をトレッドミル多段階運動負荷法による運動耐容量およびdouble productの両面から検討した。

Trapidil is known as a coronary vasodilating agent as well it has cardiac stimulating action, which is not inhibited by beta-blocking agent. But this effect dose not apear obviously in clinical cases. To elucidate this action, we treated ischemic heart patients with propranolol initially and added trapidil. Their cardiac function were estimated with noninvasive method using UCG and STI, exercise tolerance test were studied using multigated treadmill test with modified Bruce's protocol.
