ペースメーカー装着症例に対するESWLの安全性

Extracorporeal Shock Wave Lithotripsy in Patients with Cardiac Pacemakers

　異なるモードの心ペースメーカー装着中の尿路結石患者2例にESWLを施行し，良好な結果を得たので報告する。2例とも，心ペースメーカーの設定を変更した後にESWLを施行した。術後，2例とも完全排石した。術中，術後にペースメーカーに異常を認めず，脈拍，血圧などの循環動態にも異常を認めなかった。従来は禁忌であったペースメーカー装着患者へのESWLも，十分な配慮をすれば安全に施行できるものと思われた。

We present two patients with cardiac pacemakers who were successfully treated for renal and ureteral stones by extracorporeal shock wave lithotripsy (ESWL) . After the modes of cardiac pacemakers were altered in both patients, ESWL was performed. Both patients were free of stones after ESWL. During treatment and at follow-up, no dysfunc-tion of the pacemakers was noted, and there were no cardiac complications. ESWL can be performed safely in patients with cardiac pacemakers when special precautions and cares are taken to the pacemakers.
