大動脈縮窄症の造影像—その形態の分析と成因の考察

Angiocardiography of coarctation of aorta:its morphology and morphogenesis

　大動脈縮窄は形態上大動脈狭部の管状狭窄ないし低形成（tubular narrowing or hypoplasia）と峡部—下行大動脈接合部の限局性狭窄（localized stenosis）が区別され，狭義には後者の限局性狭窄を大動脈縮窄（Coarcta—tion,COA）と呼ぶのが一般的である^1〜3）。本症についてば従来胎生期循環の研究とCOAの組織学的研究から成因の研究がすすめられた。即ち動物を用いての胎生期血流の研究^4）やヒト胎児・新生児大動脈弓の計測^4〜6）から，生理的に大動脈峡部は胎生期に低血流であり，それに相応して低形成であることが明らかにされ，この胎生学的事実をふまえてCOAにともなう大動脈峡部低形成は胎生期の上行大動脈への血流低下による低形成の存続であることが，合併心奇形の研究から多くの例でほぼ確実と考えられるに至った^7〜11）。限局性狭窄形成についてはSkodaの古典的学説^12）以来動脈管組織の下行大動脈内分布とその収縮によるとの説をめぐり賛否両論があり^11〜15），Skoda説以外にも，胎生期上行大動脈血流低下こともなう分岐点説^8,16），第4弓と第6弓の接合部に生じる狭窄説などがある。

Angiocardiographic morphology and associated cardiac anomalies of coarctation of the aorta (COA) were analysed in the light of recently advanced theory of its morphogenesis. Analysis of associated cardiac anomalies in 154 clinical cases of COA revealed evidences of associated anomalies that could obstruct fetal blood flow to the ascending aorta in many cases. Systolic left ventricular outflow tract was significantly narrow in ventricular septal defect (VSD) associated with COA in comparison to the isolated VSD (p<0.01)

Good quality aortograms in 41 cases of COA were analysed.
