心室中隔欠損手術後の進行性肺血管閉塞性病変

Progressive pulmonary vascular obstructive disease following surgical closure of ventricular septal defect

　肺高血圧をともなう心室中隔欠損症の手術成績は従来必ずしもよくなかったが，最近では肺体血流比が1.3以上あり肺体血管抵抗比が0.75以下であればほぼ安全に手術できるようになった^1）〜13）。しかし手術例がふえるにつれて手術後数年で肺血管抵抗が増大し著しい右心不全を生じて死亡する例のあることが判明して来た^{14）〜16）}。この論文では5名の手術後の進行性肺血管閉塞による死亡例を報告し，手術適応について考察する。このような手術後遠隔期の進行性肺血管閉塞例は我国ではまだほとんど報告がないが，今後手術例が増加するに従って増加すると思われる。

Five cases out of 436 patients with ventricular septal defect who survived intracardiac repair developed progressive pulmonary vascular obstructive disease with fatal outcome. Ages of surgical repair in these 5 cases were 7, 8, 12, 15 and 31 years. Preoperative study revealed pulmonary to systemic resistance ratio of 0.4 to 0.6, and combined ventricular hypertrophy on EKG. Large defects ranging 1.5 to 3 cm in diameter were closed with patches and systolic pulmonary arterial pressure fell to 50 to 80 mmHg on the operations table.
