Five cases out of 436 patients with ventricular septal defect who survived intracardiac repair developed progressive pulmonary vascular obstructive disease with fatal outcome. Ages of surgical repair in these 5 cases were 7, 8, 12, 15 and 31 years. Preoperative study revealed pulmonary to systemic resistance ratio of 0.4 to 0.6, and combined ventricular hypertrophy on EKG. Large defects ranging 1.5 to 3 cm in diameter were closed with patches and systolic pulmonary arterial pressure fell to 50 to 80 mmHg on the operations table.
