要旨●炎症性腸疾患（IBD）の女性患者は治療中に妊娠・出産可能な年代を迎えることが多い．母体のIBDが寛解状態にあることが妊娠・出産の良好な転帰をもたらすとされ，妊娠前にIBDを寛解させることが推奨されている．本邦で保険適用されているIBD治療薬の多くは治療による有益性が投薬による有害性を上回るため，原則的に妊娠中も治療継続が可能であり，出産後の授乳も可能なものが多い．IBD治療薬への正しい知識がないがために胎児への悪影響を気にして治療を中止し再燃することは避けなければならない．IBDの女性患者にはIBDにおける妊娠や薬物療法の考え方を妊娠前から説明しておくことが重要である．

　Female patients with inflammatory bowel disease（IBD）often reach their childbearing years while undergoing treatment. It is recommended that IBD be in remission before conception as it is associated with favorable pregnancy and childbirth outcomes. Most IBD medications covered by insurance in Japan can be continued during pregnancy and generally permit breastfeeding after delivery, as the benefits of treatment outweigh the potential risks. It is crucial to discuss the implications of pregnancy and drug therapy with female patients with IBD before conception.
