要旨●患者は50歳代，男性のHIV感染者．CD4低値のため，上部消化管内視鏡検査を施行したところ，胃噴門部〜胃体上部小彎に不整形のびらん，胃体下部大彎に小発赤隆起，胃前庭部小彎には打ち抜き潰瘍といったさまざまな形態の病変を認め，いずれの生検からも免疫組織化学的にサイトメガロウイルス（CMV）陽性細胞を認めたため，CMV胃病変と診断した．

　A male patient in his 50s presented with HIV infection. Upper gastrointestinal endoscopy performed due to a low CD4 count revealed lesions of various shapes, including irregular ulceration from the cardiac region to the lesser curvature of the upper gastric body, small red bumps in the greater curvature of the lower gastric body, and punched-out ulcers in the lesser curvature of the antrum. Biopsies of all these lesions also showed CMV（cytomegalovirus）-positive cells during immunohistochemistry analysis. The patient was consequently diagnosed with CMV-associated gastric lesions.
