SSA/Pの分類は必要か

Necessity of the Classification “Sessile Serrated Adenoma/Polyp” ?

要旨　SSA/P（sessile serrated adenoma/polyp）は病理組織学的にHP（hyperplastic polyp）に類似しているが，構造異型や細胞増殖帯の異常などから，HPとは独立した病変として認識され，右側結腸に好発し，serrated-neoplastic pathwayを介するMSI陽性大腸癌の前癌病変として注目されている．しかし，その病理組織診断基準が明確でないため，混乱が生じている．筆者らの経験では，幼若化したHPの鋸歯状腺管がTSA（traditional serrated adenoma）様に好酸性となり，加えて若干の構造異型を伴うもの，すなわちHP with young epitheliumと診断すべき病変はTSAとの鑑別が困難なことがあり，その結果SSA/Pと診断されている印象を受ける．鑑別困難な病変に対する逃げ道的な診断であるSSA/Pは，基本的に必要のない分類と考えているが，診断がつけられない病理医には必要であろう．

　SSA/P（sessile serrated adenoma/polyp）is similar to HP（hyperplastic polyp）in its pathologic features, but SSA/P have mildly structural atypia, abnormality of cell proliferation, and other characteristics. SSA/P is paid attention to as a precancerous lesion of the MSI positivity cancer of the right side colon which is the site preferred as the serrated-neoplastic pathway, but confusion is caused in Japan because the pathological diagnostic criteria of SSA/P are not clear. We have the impression that a lesion such as the hyperplastic polyp with young epithelium having mildly structural atypia and eosinophilic cytoplasm like traditional serrated adenoma can be considered to be SSA/P. We think the classification“sessile serrated adenoma/polyp" is basically unnecessary, but is necessary for pathologists who cannot distinguish.
