・Microsporum canis(M. canis)は人獣共通皮膚糸状菌である.とくにイヌやネコなどのペットから感染することが多い. ・ヒトの頭部に感染した場合は,頭部浅在性白癬やケルスス禿瘡を呈する.しばしば脂漏性皮膚炎などと誤診され,ステロイド外用により症状が悪化する. ・頭部の膿疱や脱毛局面を診察する場合はケルスス禿瘡を念頭に置いて,KOH直接鏡検や真菌培養を実施し,ペットの飼育歴などの問診を行う必要があると考えた. (「症例のポイント」より)
A case of kerion celsi caused by Microsporum canis

Abstract A four-year-old girl visited our department for alopecia of 2-month history. She had been treated with topical steroids and oral antiallergic drugs in other dermatological clinics. Physical examination showed redness, erosion, crusts, scales, and many pustules on alopecia of the right head. We carried out KOH examination and Sabouraud’s medium culture and slide culture. Isolated fungi from the patient and a cat in her home were identified to be Microsporum canis with morphological features and direct sequencing analysis. We diagnosed this patient to be kerion celsi due to Microsporum canis from the owned cat. Three-week therapy of oral itraconazole at a dose of 5 mg/kg was effective.
