・2016年9月から11月にかけて当院で16例をコクサッキーウイルスA6型(CVA6)による手足口病と診断した. ・そのうち6例に手掌足底に皮疹を認めず,口腔内の症状も軽度であるという,非典型的な症例を認めた. ・皮疹の分布は口囲,四肢から臀部に顕著な症例が多く,個疹は浮腫性紅斑を呈し水疱も大きかった. ・手足口病発症以前より有していた湿疹部位や間擦部に,手足口病の皮疹が強く集簇する傾向にあった. (「症例のポイント」より)
Hand, foot and mouth disease due to coxsackievirus A6 with atypical eruption: Cases where no eruptions were seen in the palms and soles

Abstract We encountered 16 cases of hand, foot and mouth disease (HFMD)in Kumamoto city, Japan, from September to November 2016. We summarized the clinical features of these cases. Six cases of the 16 cases showed no eruptions on the palms and soles. The patients presented with reddish vesicles, mostly distributed in the extremities, buttocks, lips, neck and cheeks. There was a strong tendency for the skin eruptions to cluster at sites of eczema that had existed even before the onset of HFMD. In some cases, a careful differential diagnosis from chickenpox, impetigo contagiosa and Kaposi’s varicelliform disease was required. We reported cases that considered symptoms of HFMD and skin eruption should be treated in mind that there are various variations.
