 A 75-year-old woman had undergone Halsted surgery for left breast cancer 30 years ago, and 1 year prior to her presentation at our institution, she developed ulceration on her chest and general malaise. Plastic surgery and cardiology specialists diagnosed a radiation-induced thoracic skin ulcer and constrictive pericarditis. After adequate wound bed preparation for the ulcer, reconstruction was performed with a latissimus dorsi musculocutaneous flap. At 1 year after the surgery, the local control was generally good, although small ulcers remained. This patient had not only a radiation ulcer but also radiation pericarditis, thus highlighting the need for caution when radiotherapy is performed on the chest. Radiation pericarditis has a high mortality rate and is a complication that plastic surgeons who treat radiation skin ulcers of the chest should be aware of.
