横行結腸の食道裂孔ヘルニアに伴って十二指腸閉塞をきたし，その後自然軽快した1例

An unusual case of duodenal obstruction due to a spontaneously released hiatal hernia of the transverse colon

食道裂孔ヘルニアは，横隔膜の食道裂孔をヘルニア門とする内ヘルニアである。胃の噴門部または底部が脱出することが多く，胃以外の臓器の脱出を伴う頻度は低い。その中でも，横行結腸が脱出し，十二指腸閉塞まできたすことは非常にまれである。また，散見される報告例はすべて外科的修復術が施行されており，自然軽快の報告例はさらにまれである。今回我々は，画像的に興味深い所見を呈しつつ，しかも閉塞の自然軽快が得られた症例を経験したので，文献的考察を加え報告する。

An esophageal hiatal hernia is an internal hernia with the esophageal hiatus of the diaphragm as the hernia portal. The stomach is most often prolapsed, whereas the other organs are less commonly prolapsed. Transverse colon prolapse, which causes duodenal obstruction, is extremely rare. Furthermore, cases of spontaneously released obstruction are rarer. Here, we describe a case of duodenal obstruction due to a spontaneously released hiatal hernia of the transverse colon. Cases of type Ⅳ hiatal hernia with spontaneous released intestinal obstruction should be noted to choose the best treatment in a clinical sites.
