骨内髄膜腫は髄膜腫のまれな亜型で，前頭頭頂円蓋部や蝶形骨が好発部位とされる^1）2）。骨内髄膜腫は大部分が造骨性変化を呈するが，時に溶骨性変化を呈することがある。また，中頭蓋窩/底の髄膜腫ではまれに神経周囲進展を呈することが報告されている^3-6）。今回我々は，中頭蓋底に発生し溶骨性変化と神経周囲進展を呈した骨内異型髄膜腫の1例を経験したので，文献的考察とともに報告する。

We report a case of intraosseous atypical meningioma in an 82-year-old woman. CT showed an osteolytic mass in the middle cranial fossa, sphenoid bone, and maxillary sinus. MRI showed that the mass had heterogeneous enhancement and extended from the right pterygopalatine fossa to the foramen rotundum, suggesting perineural spread. Intraosseous meningioma is a differential diagnosis in osteolytic tumors of the sphenoid bone, and perineural spread may be a critical finding in meningiomas of the middle cranial base.
