Chronic active Epstein-Barr virus infection(CAEBV)is a disease characterized by chronic mononucleosis-like symptoms and elevated serum EBV-DNA copies. While various complication is reported such as interstitial pneumonia and coronary aneurysms, multiple pulmonary nodules are relatively rare findings. We report a case of an 11-year-old girl with 22q11.2 deletion syndrome who developed CAEBV and presented multiple pulmonary nodules. Although the pulmonary nodules shrank with treatment, multi-organ failure progressed slowly, and she died 4 months after admission. Radiologists must be aware that CAEBV can cause diverse complications.
