重症COVID-19肺炎には,しばしば侵襲性肺アスペルギルス症を合併することが知られており,COVID-19 associated pulmonary aspergillosis(CAPA)と呼ばれている。今回我々は,COVID-19肺炎治療経過中に,空洞性病変や多発結節を生じ,侵襲性アスペルギルス症(invasive pulmonary aspergillosis:IPA)の合併を疑ったCAPAの1例を経験したので,報告する。
COVID-19 associated pulmonary aspergillosis(CAPA)is a known fungal infection associated with severe COVID-19 pneumonia and is frequently resulting in worse clinical outcomes. COPD and long-term steroid use have been reported as risk factors. As CAPA is a highly fatal complication, early diagnosis and treatment are important if suspected, although imaging findings are non-specific. In this report, we describe a case of CAPA that could be diagnosed by detection of cavitary nodules on CT.
