腎血管筋脂肪腫の治療中に腎動静脈奇形が発見された1例

A case of renal arteriovenous malformation incidentally detected during embolization of renal angiomyolipoma

腎血管筋脂肪腫は，血管，平滑筋，脂肪組織が様々な割合で混在する良性の非上皮性腫瘍であり，サイズの大きな場合は破裂する危険性がある。一方，腎動静脈奇形は，毛細血管を介さない短絡を有する血管奇形で，cirsoid typeとaneurysmal typeに分類される。cirsoid typeではaneurysmal typeと比べ，肉眼的血尿がみられることが多いとされる。今回，腎血管筋脂肪腫の治療中に，偶発的に異なる部位に腎動静脈奇形を認めた症例を経験したので報告する。

A 73-year-old female presented with macroscopic hematuria and right lower abdominal pain. Contrast-enhanced CT images demonstrated angiomyolipoma in the right kidney. Renal angiography incidentally revealed arteriovenous malformation in the renal sinus. The angiomyolipoma was embolized with a mixture of absolute alcohol and iodized oil at 7：3 volume ratio, and the arteriovenous malformation was embolized with absolute alcohol.
