腎動静脈瘻を伴った腎血管筋脂肪腫

A CASE OF RENAL ANGIOMYOLIPOMA WITH ARTERIOVENOUS SHUNTING

　患者は73歳，女性．主訴は左側腹部痛，CTにて左腎腫瘍を認め，血管造影では動脈の動脈瘤様拡張，および著明な動静脈瘻を認めた．左腎摘出術施行，病理診断は腎血管筋脂肪腫であった．腎動静脈瘻の欠如は血管造影上，腎血管筋脂肪腫の特徴的所見の一つとされているが，本邦では510例余りのうち1％強の7例に腎動静脈瘻の存在することが判明した．

A case of angiomyolipoma of the kidney in a 73-year-old woman with the chief complaints of left flank pain was reported. Generally, arteriovenous shunting is very rare in renal angiomyolipoma, however, in this case we found remarkable arteriovenous shunting in hilar area of left kidney. This is the seventh case of angiomyolipoma with arteriovenous shunting in Japanese literature.
