自然破裂した腎血管筋脂肪腫

Spontaneous rupture of the renal angiomyolipoma：A case report

　症例は35歳，女性。右側腹部痛，嘔気・嘔吐を主訴に受診した。来院時ショック状態であった。緊急CTにて右腎周囲に血腫および右腎に1cmの動脈瘤を伴う5.5cmの腫瘍を認めた。エコーでは腫瘍は高エコーを呈していた。以上の所見より，右腎血管筋脂肪腫の自然破裂と診断した。同日，緊急腎動脈塞栓術を施行した。塞栓終了後より全身状態は安定した。腎血管筋脂肪腫は突然破裂し，多量の出血をきたした場合，生命に危険が及ぶこともある。CTによる迅速な診断と治療（動脈塞栓，腎保存手術，腎摘除術）が必要である。

A 35-year-old woman was referred to our hospital with the complaint of left flank pain, nausea and vomiting. She fell in a state of shock in the hospital. Emergency computed tomography（CT）scan showed the perirenal hematoma and 5.5cm mass with 1.0cm aneurysm in the right kidney. The mass had a fat-level low-density area. On ultrasonography the mass was hyperechoic. We reached a diagnosis of spontaneous rupture of right renal angiomyolipoma. Transcatheter arterial embolization（TAE）was performed. After the embolization, her overall status was stable.
