患者は41歳，男性。左側腹部痛を主訴に救急外来を受診した。腹部CTにて，左腎上極に8cm大の腎血管筋脂肪腫と腎周囲に広がる血腫を認めたため，左腎血管筋脂肪腫の自然破裂による出血と診断し，経動脈的塞栓術（transcatheter arterial embolization，以下TAE）を施行した。塞栓物質には，無水エタノールとリピオドールを7：3に混合した液を使用した。TAE後の血管造影では，左腎上極の腫瘍部分は造影されず下半分のみが選択的に造影された。現在，腫瘍の増大，再出血などを認めず無症状で経過している。

A 41-year-old man visited our hospital complaining left sever flank pain. Computed tomography（CT）scan showed rupture of angiomyolipoma（AML）of the left kidney. Transcatheter arterial embolization（TAE）of the AML was performed by injection of ethanol and lipiodol. The postembolization angiogram showed devascularization of the AML with patent blood flow to normal renal parenchyma. The patient has been followed up for 4months with reduced tumor size and no re-rupture or re-bleeding. We think TAE to be recommended for patients with renal AML rupture as a first-line treatment.
