はじめに
本術式は，頸椎後縦靭帯骨化症（ossification of the posterior longitudinal ligament：OPLL）に対する骨化切除術の危険性を顧みて，現在の九段坂病院山浦伊裟吉顧問が提唱した7）．すなわち，骨化を菲薄化した後に前方に浮上させることによって脊髄除圧を図る術式である．脊髄および脊髄前角への直接的な圧迫は解除され，さらに前脊髄動脈の血行改善が期待できる．
よい適応は，骨化占拠率が50〜60％を超える厚い骨化あるいは頸椎の側面アライメントが後弯を呈する場合である．本稿では，その基本的な手技とピットフォールへの対処について述べ，最近の工夫について紹介する．
