はじめに

　術中脊髄モニタリングは，術中に脊髄の中枢，または末梢を刺激することで，脊髄伝導路を介した誘発電位の変動を監視する機能診断法である．中でも経頭蓋電気刺激運動誘発電位（transcranial motor evoked potential：Tc-MEP）は運動路を監視することができ，ほかの脊髄モニタリング法である体性感覚誘発電位や経頭蓋電気刺激脊髄誘発電位と組み合わせることでさらに高感度のモニタリングが可能となっている．そこで本項では，脊髄モニタリングの中でもTc-MEPに関する一般的な知識と脊柱変形手術，後縦靭帯骨化症（ossification of the posterior longitudinal ligament：OPLL）手術における術中脊髄モニタリングの実際について，われわれが行ってきた研究結果を中心に述べる．
