Guidelines for surgical management of acute cholecystitis

　急性胆管炎と胆嚢炎の診療ガイドラインとなる「科学的根拠に基づく急性胆管炎・胆嚢炎の診療ガイドライン」が発売された．これにより急性胆嚢炎に対して科学的根拠に基づく治療法が示され，広く適切な治療がなされることが期待される．しかし，急性胆嚢炎患者のすべてがガイドラインの治療法にあてはまるわけでないことを理解しなければならない．その前提において，急性胆嚢炎に対する胆嚢摘出の術式については，治療にあたる外科医の得意な術式を選択することが重要であることを示し，可能ならば腹腔鏡下胆嚢摘出術が望ましいことを示した．その適切な手術時期については，入院後早期の胆嚢摘出術が望ましいことを示した．一方，急性胆嚢炎に対するガイドラインに記載された治療法とは別に標準的治療法という概念があり，ガイドラインにおける治療法のみが急性胆嚢炎の治療法でないことを概説する．

Evidence-based guidelines for the management of acute cholangitis and cholecystitis have been published. The purpose of the guidelines was to promote further understanding of the diseases for doctors. The patients with acute cholecystitis should receive benefits from these guidelines. In the guidelines, the recommended sur-gical procedure for patients with acute cholecystitis should be based on the surgeon's experience although laparosocopic cholecystectomy is prefered when possible.
