急性胆嚢炎に対する待期手術における腹腔鏡下胆嚢摘出術困難例の予測と対策

Surgical Difficulities in Delayed Laparoscopic Cholecystectomy for Acute Cholecystitis: Its Predictive Factors and Timing of Conversion to Open Surgery

　自験の急性胆嚢炎は42例で，うち31例に腹腔鏡下手術を行ったが，すべてが待期手術であった．8例が途中で開腹に移行し，移行率は25.8％であった．腹腔鏡下胆嚢摘出術例と開腹移行例を比べると，後者に術前の超音波検査での胆嚢壁肥厚例や胆嚢造影陰性例が多い傾向があった．手術の時期は患者の状態に合わせて選択するのが基本と考えるが，時期によって炎症の病態が異なるので，その病態に合わせて手術を進めるべきである．特に待期手術では，胆嚢周囲の線維化と血管増生による出血に注意する必要がある．開腹移行を事前に予測することは難しいが，胆道損傷を疑った場合や手術の継続が困難と判断した場合は，無理せず開腹移行すべきである．

Although laparoscopic cholecystectomy (LC) has been established as the standard procedure of choice for the treatment of cholelithiasis, the procedure may be difficult and challenging in some patients with acute cholecystitis. We initiated the present study in order to clarify the question of safety of LC in the presence of the various types of gallbladder inflammation. Among 1127 patients in whom laparoscopic and open cholecystectomy (OC) was performed in our institute from February 1991 to February 1998, 42 (3.8%) had acute cholecystitis. LC was attempted in 31 patients with acute cholecystitis.
