急性胆嚢炎に対する腹腔鏡下胆嚢摘出術（LC）は困難例が多いといわれているが，自験例の急性胆嚢炎に対するLC 107例（6.8％）中，早期手術27例には待期例に比べ困難例が少なく，重篤な合併症や開腹移行が認められなかった．この経験をもとに急性胆嚢炎の治療方針を示し，安全にLCを施行するための手技の工夫と問題点を述べた．急性胆嚢炎発症後5日以内は早期手術を第一選択とする．5日以上経過例はPTGBDを施行する．剥離困難例には胆嚢部分切除も有効である．急性胆嚢炎に対する早期LCは手技の習熟と工夫により良好な手術成績が得られ，首尾よく行えば患者にも多くの利点がもたらされると考えられた．
The role of laparoscopic surgery for acute cholecystitis is still controversial because of the high incidence of complication and conversion to open surgery. We performed laparoscopic cholecystectomy in 107 patients with acute cholecystitis, and early surgery was performed in 27 of 107 patient. Among the early surgery group, there was no mortality, no major complication, nor conversion to open surgery. Performing cholecys-tectomy within 5 days of presentation is safer and easier than cholecystectomy performed after 5 days. The point for safe cholecystectomy is identification of the cystic duct.
