急性胆嚢炎に対する待機的腹腔鏡下胆嚢摘出術の位置づけ

Role of delayed laparoscopic cholecystectomy for acute cholecystitis

　腹腔鏡下胆嚢摘出術（laparoscopic cholecystectomy：LC）が導入され10年以上を経た現在，かつては禁忌であった急性胆嚢炎に対しても積極的にLCが施行されている．さらに最近発表された「急性胆管炎・急性胆嚢炎の診療ガイドライン」では早期LCが推奨されているが，実際の臨床の現場では重症度判定やLCの適応（早期vs待機）については依然不明な点が多い．自験例の検討では，待機LCは急性胆嚢炎による高度肝機能障害を有する症例に施行されており，早期LCと比較して総入院期間は延長するものの術後在院期間は同等であり，手術時間はむしろ短縮する傾向がみられた．これらのことから肝機能障害を有する症例や炎症が高度な症例においては，症例の個々の病態に応じた（オーダーメイド）治療として早期LCに固執することなく，保存的加療を行ったうえで待機LCを行うことも重要な選択肢になりうると考えられた．

Background: The management of acute cholecystitis has evolved with the increasing experience of laparoscopic cholecystectomy in the past decade. However, there are still controversies over the use of laparoscopic cholecystectomy to treat acute cholecystitis. The aim of this study was to clarify the optimum man-agement between early and delayed laparoscopic cholecystectomy for patients with acute cholecystitis.

Patients and methods: A retrospective analysis of 32 patients with acute cholecystitis who underwent laparoscopic cholecystectomy from 1991 to 2003 was conducted.
