ウルトラリトラクター®を使用した内視鏡下甲状腺切除術の1例

Endoscopic thyroidectomy using Ultra-retractor®

　甲状腺濾胞腫瘍に対し，ウルトラリトラクター^®を用いて内視鏡下甲状腺切除術を施行した，鎖骨下30mmの皮膚切開のみですべての操作が可能で，甲状腺右葉下極に存在した23mmの腫瘍を切除した．手術時間87分，出血量50mlであった．ウルトラリトラクター^®の視野は通常手術とほぼ同様で，鉗子類とは位相が異なるため競合せず，フードにより組織との接触がなく，良好な視野を保ちつつ円滑に操作を行いえた．術中偶発症，術後合併症を認めず，術後経過も良好であった．医療経済的には問題点を指摘されるが，内視鏡下甲状腺切除術には有用であると考えられた．

We performed endoscopic partial thyroidectomy using Ultra-retractor^® with a 30 mm skin incision under the clavicle. A 23 mm tumor, located in the right lobe of the thyroid gland, was resected in 87 minutes with 50 ml of blood loss. This device provided a good visual field and easy way of devicehandling. Furthermore, the endoscopic forceps didn't conflict with the Ultra-retractor, as they could be used in the different altitude. The patient was discharged from the hospital in 4POD without any periand postoperative events. This device is expensive but useful for safely performing endoscopic thyroidectomy.
