注腸造影検査中のS状結腸穿孔性腹膜炎に対して腹腔鏡補助下手術を施行した1例

Successful laparoscopy assisted repair of perforation of the colon sustained during barium enema X-ray of the large intestine

　大腸穿孔時の緊急手術としては開腹手術が選択されることが多い．今回，注腸造影検査中に併発したS状結腸憩室穿孔に対して腹腔鏡補助下に治療できた症例を経験したので報告する．症例は74歳の男性．CEA高値の精査目的に注腸造影検査を施行した，検査中に突然大腸が虚脱し，腹腔内にバリウムと多量の空気の漏出がみられた．直前の造影結果から，S状結腸憩室部の穿孔と診断され，外科に紹介された．穿孔から約3時間後に手術を開始した．穿孔から短時間であること，注腸造影検査の前処置により便がほとんどないこと，バリウムの貯留が骨盤内に限局していたことから腹腔鏡下手術で治療可能と考え，腹腔鏡補助下に穿孔部の縫合閉鎖と十分な腹腔内洗浄・ドレナージを行った．術後経過は良好で，元気に退院した，大腸穿孔による腹膜炎でも，適応を選び，徹底した腹腔内洗浄を行うことで腹腔鏡下手術により治療可能であり，有効な方法と考えられた．

Immediate open surgery has been recommended for the management of perforation during colon examina-tion. Laparoscopic closure for perforation of diverticulum of the sigmoid colon caused by barium enema x-ray of the large intestine is described. A 76-year-old man was found to have perforation of sigmoid colon diverticulum during barium enema x-ray of the large intestine. We performed laparoscopic repair after having an informed consent.
