バセドウ病に対する内視鏡下甲状腺亜全摘術の侵襲性および患者QOLの評価—頸部切開手術との比較

Evaluation of Surgical Invasiveness and Quality of Life of Endoscopic Subtotal Thyroidectomy for Graves' Disease: Comparison with Open Thyroidectomy

　バセドウ病に対する内視鏡下甲状腺亜全摘術の手術侵襲について検討した．対象はバセドウ病手術症例25例で，内訳は内視鏡下甲状腺亜全摘術群15例，頸部切開下甲状腺亜全摘術群10例とした．両群間で手術成績，臨床検査結果（白血球数，C-reactive protein），サイトカイン（inter—leukin−6,interleukin−1ra）値の変動に差はなかった，創部皮膚の感覚検査では，退院時において内視鏡下群で前頸部，前胸部に軽度の知覚低下を認めたが，術後6か月目には全例で改善がみられた．術後のアンケート調査では，内視鏡下群で手術に対する満足度は高かった．内視鏡下甲状腺亜全摘術の手術侵襲は従来の頸部切開下手術と同等であったが，整容的な面での満足度が高く，有用性の高い術式であると考えられた．

The surgical invasiveness was compared for two types of subtotal thyroidectomies for Graves' disease: endoscopic thyroidectomy (ET) and traditional open thyroidectomy (OT). The study subjects included 25 pa-tients (ET. n=15 OT, n=10) with Graves' disease who received subtotal thyroidectomies between Feburary 1997 and August 2000. The recorded surgical parameters and postoperative laboratory data showed no signifi-cant differences between the two groups.
