前胸部アプローチ法による内視鏡下甲状腺切除術

Endoscopic Thyroidectomy with Precordial Approach

　甲状腺疾患における外科手術後の手術瘢痕は美容上の問題点となり，頸部の瘢痕収縮に伴う違和感を訴える患者も少なくない．われわれは，32歳，女性の甲状腺腺腫に対して，前胸部アプローチ法により本邦で初めて内視鏡下甲状腺手術を施行した．広頸筋下層の鈍的剥離と超音波切離装置により良好な視野が確保され，手術は安全に行われた．顔面の皮下気腫や縦隔気腫を認めず，術後の疼痛も軽微で，反回神経麻痺，出血などの合併症もなかった．頸部の美観は良好に保たれ，患者は第7病日に退院となった．本術式は従来の甲状腺手術に遜色のない切除が可能であり，今後さらに適応が拡大するとともに，甲状腺外科領域における選択術式の1つとなることが期待される．

Recently, endoscopic surgery has been applicable to cervical exploration. We have developed new tech-niques for endoscopic thyroidectomy with precordial approach in a 32-year-old woman with follicular adenoma (5 cm in diameter) of the thyroid gland. Three trocars (A 12 mm midline trocar for endoscope, 5 and 12 mm lateral trocars) were inserted from the anterior chest wall to avoid incisions on the neck. A subplatysmal emphysema was created with CO₂, insufflation at 6 mmHg. The inferior thyroidal vessels and the parenchyma of the gland were easily dissected and divided by an ultrasonically activated scalpel.
