卵管形成術—卵管留症と卵管閉塞

　卵管形成術は体外受精の普及に反し古典的な治療になりがちであるが，近年の腹腔鏡下手術の進歩によって腹腔鏡下卵管形成術が可能になったことから新たな展開が期待される．卵管形成術には子宮卵管移植術，癒着剥離術，卵管端々吻合術，卵管開口術卵管采形成術の5型がある．腹腔鏡下卵管形成術は，開腹術による卵管形成術に比べ手術侵襲が小さく，患者に負担が少ないことばかりでなく，多くの利点があることから患者に受け入れられやすい治療法である．

　卵管性不妊のなかで卵管留症と卵管閉塞は最も頻度の高い不妊原因である．しかし，これらを無視して体外受精を行っても必ずしもよい結果が得られるとは限らない．ここでは，これらの対処法について述べる．

In vitro fertilization and embryo transfer (IVF & ET) has become a widely accepted modality, and in Japan, there are roughly 500 facilities which offer this service. In contrast, Tuboplasty which has a history longer than IVF & ET, has not shown a wide spread popularity. However, with the improvement of laparoscopic techniques, laparoscopic tuboplasty has become possibil and this method may become popular.
