進行S状結腸・直腸S状部癌に対する腹腔鏡下D3郭清の手術手技と成績

Technique and results of laparoscopic D3 lymph node dissection for advanced cancer of the sigmoid colon and rectum

　大腸癌に対する腹腔鏡下手術は各施設で適応や手術方法に差があり，標準術式が確立されていないのが現状である．そこで進行S状結腸・直腸S状部癌に対する，当教室の標準術式であるIMA根部・LCA・自律神経温存D₃郭清の手術手技と成績を報告する．3D-CT血管画像で術前にシミュレーションを行うことによってIMA根部の高さ，LCAの分岐位置が予想でき，安全に郭清操作を行うことができた．手術成績，臨床経過，性機能も良好で，低侵襲でしかもQOLの向上が得られたと考えている．術後観察期間中央値14.5か月の成績は肝転移1例であり，全症例生存中である．IMA根部・LCA・自律神経温存D₃郭清は，長期成績に関しては術後観察期間もまだ短く慎重な観察が必要であるが，短期成績および機能温存に関しては満足できる結果であった．

Surgical procedures and indications for laparoscopic surgery in the treatment of colon cancer (vary depend-ing on the institutions), and no standard technique has been established yet. For the treatment of advanced can-cer of the sigmoid colon or rectum, we present a laparoscopic D₃ lymph node dissection technique, conserving the IMA, LCA and autonomic nerve and discuss the usefulness of this technique. By preoperative simulation based on 3D-CT data, the height of the root of the IMA and the location of the LCA bifurcation could be pre-dicted, making it possible to perform lymph node dissection (safely).
