3D CTナビゲーションシステムを用いた内視鏡下胃癌リンパ節郭清

Laparoscopic lymphnode dissection for advanced gastric cancer using the 3D CT navigation system

　腹腔鏡下胃癌リンパ節郭清は手技の複雑さや止血の困難さから，高度な手技が求められる．特に進行胃癌に対しては血管を温存したリンパ節郭清が必要であり，血管系の解剖把握が重要である．最近のCT画像処理技術に着目し，造影CTを内視鏡画像と同じ視野に変換し，さらに内視鏡の動きにあわせてリアルタイムに変換することで，あたかも血管を透見しながら手術ができるシステムを開発した．これにより血管系を探る操作がなくても手術を進めることができるため，より安全で精度の高い郭清が可能となった．本稿では，このシステムの概要について述べる．

Laparoscopic lymphnode dissection for advanced gastric cancer demands a high level technique, because con-trolling bleeding and preserving the vessels while dissecting the lymph node (LN) are difficult. Playing an im-portant role in this complicated procedure is the knowledge for the anatomy. We have developed a novel image information system to provide 3D CT images reconstructed as laparoscopic views and synchronized with the laparoscopic motion.
