超音波凝固切開装置の腹腔鏡下肝切除術への応用

The Application of Ultrasonically Activated Scalpel for Laparoscopic Hepatic Resection

　最近の腹腔鏡下手術の進歩は著しく，胆嚢摘出術や腸切除のみならず肝切除にも応用されるに至った．しかしながら，肝切除においては技術的な難易度が高く，その適応は腫瘍の局在や腫瘍の大きさに規定される．したがって，現時点においてはその手技的な問題点さえ解決されるならば，開腹下の肝切除術に劣らない治療法として，その低侵襲性と合わせ根治治療としても十分に期待できる．この手技的な問題点を解決する1つの手段として，肝切除の際の超音波凝固切開装置の応用がある．肝切除のような実質臓器の切離に関しては，その使用方法と適応に十分注意すれば，内視鏡下手術や小切開肝切除などへの応用の可能性は十分にある．

Laparoscopic surgery has rapidly become widely used in the past several years and its indications have become wider as laparoscopic surgical techniques have improved. Recently, this type of operation has been applied the hepatic resection for the malignancies as well as benign tumors. However, laparoscopic hepatic resection is difficult and its indication is limited by the localization and size of tumors. If the problem of this difficulty can be resolved, this procedure would be the proper therapy because of its curability and low grade injury.
