腹腔鏡補助下幽門側胃切除術における術中偶発症，術後合併症の予防とその対策

Prevention of intraoperative incidents and postoperative complications in laparoscopy-assisted distal gastrectomy

　腹腔鏡補助下幽門側胃切除術（LADG）が導入されて10年あまりになるが，近年，適応の拡大とともに早期癌を中心に各施設で導入が進み，その手術症例数は増加傾向にある．新しい手技の導入による低侵襲手術症例の増加とあいまって，術中偶発症，術後合併症の増加も危惧される．これらを回避しLADGを安全に導入するためには，そのピットフォールとトラブルシューティングについて十分認識することが重要と思われる．本稿では出血，他臓器損傷，再建に関するトラブルを中心にLADGにおける術中偶発症，術後合併症とその対策について述べる．

Since laparoscopy-assisted distal gastrectomy (LADG) was introduced over ten years ago, the operated cases have increased in various institutions. As the number of operations using this new technique increases, there is always the possibility of increased intraoperative incidents and postoperative complications. It is very important to recognize the pitfalls and troubleshootings in LADG in order to avoid incidents and complications. Here, we will report events of intraoperative bleeding, injury to other organs and problems with reconstructions as well as their countermeasures.
