バルーンカテーテルを用いた内視鏡下筋膜下穿通枝切離術（SEPS）—手術手技の手順とポイント

　静脈うっ滞性の潰瘍や皮膚炎の治療において，内視鏡下筋膜下穿通枝切離術（sub—fascial endoscopic perforator surgery）が広く行われるようになってきた．われわれはCEAP分類のclass 4〜6の症例を手術の対象としている．不全穿通枝の同定には超音波検査を行っている．手術はバルーンカテーテルを用いて筋膜下を剥離し，炭酸ガスを送気することで筋膜下腔を拡張させ術野を確保し，カメラ用と鉗子用の2か所のポートを留置して行っている．大伏在静脈のストリッピングは必要に応じて行っている．手術症例は全例が合併症もなく症状の改善がみられており，この術式は有効な治療法と考えられた．

Subfascial endoscopic perforator surgery (SEPS) has become popular in the treatment of a stasis ulcer in re-cent years. We have performed SEPS to the patients with class 4-6 (CEAP). We identify incompetent perforat-ing veins by ultrasonography. The cannula with a balloon is inserted into a subfascial space and the balloon is inflated to distend it. Carbon-dioxide gas is insufflated into the subfascial space to improve visualization. Two ports is placed on the lower part of a leg.
